This section is relevant to clients who are beginning therapy with me.

Please download, fill in and return to me both (1) the relevant contract and (2) the client information sheet. You can return them to me via email at cj@rainorshinetherapy.co.uk. Please also read the policies that apply to you from the ‘policies’ section below.

This is the document new clients fill in and send back to me. The information provided allows me to support you in a medical emergency and to find out about your access needs. This file does not open in a new browser-rather it should open a pop-up below which you can click on to download it. .
